Understanding Risk in Gay and Bisexual Men

Sexually transmitted infections (STIs) are biologically the same regardless of sexual orientation. However, different sexual practices, including receptive anal sex, rimming, fisting, and chemsex, may alter levels of risk and the types of infections seen more commonly in men who have sex with men (MSM).

Certain infections remain particularly relevant within the LGBTQIA+ community and require tailored screening and prevention strategies.

HIV

HIV continues to disproportionately affect gay and bisexual men. The introduction of PrEP, wider access to testing, and early treatment have significantly reduced new infections in London and across the UK.

Effective treatment means that individuals with an undetectable viral load cannot pass HIV on to sexual partners (U=U). However, there is still no cure, and regular testing remains essential.

We provide HIV testing, PrEP advice and prescribing, and PEP guidance where appropriate.

Hepatitis C

Hepatitis C can be sexually transmitted, particularly where there is exposure to blood. Risk increases with traumatic unprotected anal sex, fisting, sharing injecting equipment, and in the presence of other STIs such as syphilis or HIV.

Routine hepatitis C screening is recommended for gay and bisexual men, especially those with additional risk factors.

Hepatitis B

Hepatitis B is more easily transmitted sexually than hepatitis C and can be spread through anal, vaginal, or oral sex.

Vaccination is strongly recommended for all gay and bisexual men and others at increased risk, including people who inject drugs.

Human Papillomavirus (HPV)

HPV commonly causes genital warts, but persistent infection — particularly in HIV-positive individuals — can lead to pre-cancerous anal changes.

We offer HPV vaccination (Gardasil) and anal smear testing to screen for early signs of anal cancer in higher-risk patients.

LGV (Lymphogranuloma Venereum)

LGV is a more invasive form of chlamydia infection seen predominantly in gay men who have receptive unprotected anal sex. It spreads more easily where there are multiple sexual partners.

Symptoms may include rectal bleeding or discharge, painful inflammation (proctitis), tenesmus (a constant urge to open the bowels), ulceration, and sometimes fever. When diagnosed early, LGV is effectively treated with antibiotics. If untreated, it can cause serious and lasting rectal damage.

MSM-Specific STI Screening

We offer bespoke STI screening tailored to gay, bisexual, and MSM patients, including:

• HIV Duo (p24) testing, acute hepatitis screening (A, B and C), throat and rectal swabs for chlamydia and gonorrhoea, rectal testing for Mycoplasma genitalium, urine screening, rectal HPV testing, and anal smears.

Many patients use our service alongside NHS care for additional reassurance, more frequent testing, or access to services not routinely available, such as HPV testing, anal cytology, or vaccination. Most results are available the same day.
